Physical Examination Form - If completed prior to registration, scan and upload during this process, otherwise once complete, please mail to your child's school.

• Affidavit of Domicile
• Copy of Executed Deed
• Signed Settlement Statement
• Copy of Executed Lease
• Affidavit of Title
• H.U.D. Settlement

Choose from THREE of the following:

• Utility Bill
• Driver's License
• Car Insurance
• Vehicle Registration
• Voter Registration
• Delivery Receipts
